Physical Activity Readiness Questionnaire

Before taking part in the DDMIX training course we want to know that you are ready and able to perform physical activity. There are aspects of this course which require physical activity and that you consent that you are able to perform these activities.Please read the following carefully and check YES or NO opposite the question if it applies to you. If you answered NO to all questions above, it gives a general indication that you may participate in this workshop. The fact that you answered NO to the above questions, is no guarantee that you will have a normal response to exercise.

Schools

DDMIX is working closely with schools to introduce DDMIX into their timetables and develop after school clubs. If you are interested in teaching in schools please inform us. Please be advised to work in schools you will have to have a DBS check performed on you.

In order to complete the DDMIX Instructor Training Course you must have one of the following qualifications or the equivalent of:· 
Level 2 Fitness Instructing e.g. Exercise to Music (ETM), Aqua, Gym, Children’s Fitness· Level 2 Group Training to Music (formerly ETM)· Level 2 Group TrainingLevel 2 (DFQ) Exercise, Movement and Dance Level 2 Gym InstructorLevel 3 Personal TrainerLevel 3 Yoga OR 300 hours certificateLevel 3 Pilates
It is the responsibility of you, the instructor, to ensure you have all the relevant pre-requisites, insurance, qualifications, tax, certifications, fitness and educational requirements, or licenses needed to deliver DDMIX classes and ensure that you comply with the laws and regulations where you conduct DDMIX classes. If you have any questions regarding any of the above please email info@diversedancemix.com.
